We are going to Centrifuge at Southwest Baptist University in Bolivar, Missouri from June 27 - July 1, 2011. There will be a Centrifuge Information meeting on Sunday, January 23, 2011 at 4 p.m. for ALL students and parents at the FBC Youth Building. A $50.00 deposit is required to reserve your spot for camp. The deposit confirms your spot at Centrifuge. All deposits must be paid by February 1. 2011.
There will be two Medical Release forms that must be signed and notarized before your student may attend camp. The First Baptist Church Medical Release form is available for download below. The Lifeway Centrifuge Medical Form will be available at a later date.
